Option 1: Do It Yourself for $19.95

For only $19.95 you can prepare a Colorado Beneficiary Deed for your property on our website and receive your deed and the supplemental forms you and your heirs will need. For this price, you will receive the following:

1. A completed Colorado Beneficiary Deed.
2. A "Revocation of Deed" form and Instruction sheet ; see Note 1.
3. A "Supplementary Affidavit of Proof of Death;" see Note 2.
4. "Instructions for Heirs."
5. A "Supplemental Affidavit of Death for a Joint Tenant" with Instruction sheet.

You should not use a beneficiary deed without these additional forms.

Upon completion and purchase of your Colorado Beneficiary Deed and Supporting Documentation, we will send you an email message with download instructions. These forms can then be printed for your records.

Note 1: The people who would most likely need to revoke their Beneficiary Deed would be those who apply for Medicaid assistance for long term care later in life. The government could require the deed's revocation as a condition of your receiving Medicaid.

Note 2: Although title to your home will legally pass to the beneficiaries you name in your Beneficiary Deed immediately upon your death, the transfer of title to the named beneficiaries must be completed on the public record as required by Colorado Revised Statute 15-15-413. To do this your beneficiaries will need to record a certified copy of your death certificate and the affidavit required by law. These forms enable them to do this easily.
